Dental Texas Children’s Medicaid and CHIP Plan
Medicaid and CHIP offers are:
Backpack Program:
Members between 3 and 19 years old who complete a dental checkup within 180 days of enrollment receive a UHC Dental Backpack.
Spin Toothbrush Program:
Members between 7 and 12 years old who complete a dental checkup within 180 days of enrollment receive a Spin toothbrush.
Sports Mouthguard Program:
Members between 6 and 19 years old who complete a dental checkup receive a sports mouthguard. 1 per year.
$25 Walmart Gift Card program:
Members between 0 months and 19 years old who complete a dental checkup within 180 days of enrollment receive a $25 gift card.
Dental Care Kit Program:
Members between 3 and 6 years old who complete a dental checkup within 180 days of enrollment receive a dental care kit.

Members from 6 through 13 years old who complete; Health Risk Assessment, and qualification and enrollment into the UnitedHealthcare Dental Case Management program. Limit 1.
Choice of either Bluetooth 3.0 Wireless Speaker or Wireless Earbuds:
Members from 14 years through 16 years old who complete; Health Risk Assessment, and qualification and enrollment into the UnitedHealthcare Dental Case Management program. Limit 1
Fitness/Activity Tracker with Bluetooth 4.0 technology:
Members from 17 years through 19 years old who complete; Health Risk Assessment, and qualification and enrollment into the UnitedHealthcare Dental Case Management program. Limit 1
Choice of either a $15 Xbox Gift Card or a $15 Hulu Gift Card:
Members enrolled in the UHC Dental Case Management Program who complete a milestone event within the program. Milestone events include receiving a preventive dental visit or completing a dental treatment plan.
