If you are now eligible to participate in the CHIP or Medicaid Program, you will be asked to choose a dental plan. If you don’t choose a dental plan, one will be chosen for you. To choose a dental plan, contact the enrollment broker Maximus by visiting yourtexasbenefits.com or by calling 1-800-964-2777 (TTY 7-1-1). Tell them you want to choose UnitedHealthcare Dental Plan of Texas. They may ask for you to choose a Dentist at that time. If you are not able to, we can help you choose one that is close to your home and will see all of your family members enrolled with UnitedHealthcare Dental Plan of Texas.
