To start, you will need to contact the Texas Medicaid and CHIP Enrollment Broker at 1-800-964-2777. Tell them you want to choose UnitedHealthcare Dental Plan of Texas. They may ask for you to choose a Dentist at that time. If not, we can help you choose one that is close to your home and will see all of your family members enrolled with UnitedHealthcare Dental Plan of Texas.

Please remember that enrollment starts on June 1, 2020, and you will be able to see your Dentist starting September 1, 2020.
